Project Objectives
The SCCV project will utilise a collaborative approach focused on working with the 9 integrated Cancer Services (ICS) to implement Victoria’s Supportive Care Policy. Taking a state-wide approach the project objectives are listed under the four strategic directions outlined in the policy:
Strategic Direction 1: Identifying the supportive care needs of people affected by cancer
- Implement evidence based strategies to assist identification of supportive care needs of the ICS population and analyse and respond to identified supportive care service gaps.
Strategic Direction 2: Capacity building for optimal supportive care
- Continue to build capacity for optimal supportive care (i.e. engage executive and clinical leadership, facilitate a multidisciplinary approach to care, support workforce training, and consumer involvement).
- Demonstrate evidence of workforce training for effective initial screening and follow up assessment (including communication skills, documentation processes).
Strategic Direction 3: Implementing supportive care screening into routine practice
- Implement and document routine use of validated supportive care screening tools and resources to facilitate identification of supportive care needs.
Strategic Direction 4: Addressing supportive care needs – referral and linkage
- Demonstrate evidence of supportive care partnerships to support referral and linkage, service development and quality improvement.
